Exact cause under investigation? Still?

In the section Cause it states the incident is under investigation. This occurred in 2012. That is six years ago now. Surely the cause was determined? I've added information about the cause to the article. If anyone wants to change the wording, please feel free. I don't know what the sources mean when they say the joints were "attacked", which is why it's in quotes. Weakened, perhaps? I honestly have no idea. Regarding the previous edit I made just before it: I was looking for how to add the section link in the edit summary and slightly forgot the differences between source editing and visual editing, and accidentally published changes.
